UI Health Antithrombosis Center

Welcome to the Antithrombosis Center at the University of Illinois Hospital & Health Sciences System (UI Health). We are a clinical team of the University of Illinois at Chicago (UIC) College of Pharmacy Department of Pharmacy Practice. We are happy that you have chosen us to provide your outpatient thrombosis care.

Under collaborative practice arrangements with physicians and other healthcare providers, we independently manage the initiation, monitoring, and follow-up of antithrombotic therapy utilizing warfarin, low-molecular weight heparins, and target-specific oral anticoagulants. In collaboration with the UI Health Personalized Medicine Program, we incorporate patient-specific genetic workups into patient care. We see patients with a wide range of thrombotic conditions, in ages ranging from the very young to the elderly.

If you are a patient newly started on an antithrombotic medication, or have been taking an antithrombotic medication and are looking for a place for antithrombotic care, we welcome you to read the following information below.

 

MAKING AN APPOINTMENT FOR NEW PATIENTS
Patients must be referred by their physician to be seen in the UI Health Antithrombosis Center. This is usually your primary care doctor. If you were recently admitted to the University of Illinois Hospital, you will likely be referred by the medical team that provided your care while you were hospitalized.

Appointments must be made in advance. There are no walk-in appointments.

For the courtesy of all patients, please call 24 hours in advance if you need to cancel or reschedule your appointment.

ACCEPTED INSURANCE PLANS
Our clinic accepts health insurance plans accepted by UI Health. Click here to see if your insurance plan is accepted at UI Health. Most insurance plans such as PublicAid, Medicare, and many private plans are accepted at UI Health.

Some insurance plans may require a referral from your primary care doctor, especially if you have an HMO plan. You should check with your insurance company.

OUR PROVIDERS
Our faculty are highly-trained clinical pharmacists that work in partnership with UI Health physicians and are regionally and nationally recognized experts in the research, management, and treatment of thrombosis. Many of our clinical pharmacists hold board certification in the clinical pharmacy specialties of pharmacotherapy and ambulatory care clinical pharmacy.
